Please, login to your 'Mijn Simyo'. -> go to 'Mijn Instellingen & Simkaart' -> look for 'Simkaart' > 'Blokkeer Simkaart'.

This will work. You can follow RicoK's instructions to block it. Then you can choose 'Aanvragen vervangende simkaart' to get a new one. It will cost you 5,- and you will receive the new simcard within 3 working days.
